Please communicate with your School Nurse to let them know about any health concerns that would impact your child's learning or safety at school. With input from you, your child's Health Care Provider, and the School Nurse's expertise in school health services, an Individual Health Plan can be developed to help your child be safe, healthy and successful at school.
Other plans that may help promote your child's health and safety at school is an Emergency Care Plan (ECP). This plan is needed if your child is at risk of needing emergency medication or is high risk for needing to be transported to the hospital for emergency care during school hours. This form can be started with your physician and the school nurse will help train the teacher and other providers in how best to care for your child.
